Create an email sign-up form so constituents can sign up for your organization's emails. After constituents submit the form, they see a success page. You can customize the form and success page.
Step 1 — Create and Design the Form
To create and design a sign-up form:
Click Communications.
Click Forms.
On the Email Sign-up tile, click Select.
Click New.
Enter the Form Name. This name appears on notification emails that Bloomerang CRM sends to your organization when a constituent submits this form.
Select a Confirmation Email.
Select a Notification Email address. When a constituent submits this form, Bloomerang CRM sends a notification to this email address.
To add up to 10 more notification emails, click Add Email.
(Optional) To gather more information about the donor, click Add Constituent Field, Add Interaction Field, or both. To reorder custom fields, click the up or down arrows. To remove a field, click the x. Required custom fields are required on the form.
Tip: Be judicious about how many custom fields you place on the form. Constituents are more likely to fill out three or fewer fields.Click Next.
Step 2 — Design the Success Page
Constituents see a success page after signing up. You can use your success page as an opportunity to thank and engage with the constituent. You can design the page in Bloomerang CRM or use your own success page.
To design a success page in Bloomerang CRM:
On the Build in Bloomerang tab, edit a page Title.
Enter the Message Body.
(Optional) To enter social media information for Twitter (X):
Enter a Twitter (X) handle. This also updates your organization settings.
Enter a Prefill Tweet Text. The constituent can personalize the message.
Select Tweet Button.
Select Follow Button.
(Optional) To enter social media information for Facebook:
Enter a Facebook URL. This also updates your organization settings.
Select Share Button. The constituent can click this button to share your Facebook page. The constituent can personalize the message.
Select Like Button. The constituent can click this button to like your Facebook page.

Step 3 — Preview and Edit the Form
To preview and edit the form:
Click Save and Preview.
To make changes to the form content:
Click Edit.
Make your changes.
Click Next.
Click Save and Preview.
Step 4 — Copy the Code Snippet
To embed this standard form on your website, copy a short JavaScript code snippet from Bloomerang CRM and paste it to your website.
To copy the code snippet for this form:
Click Save and Preview.
Click Next.
Copy the code snippet.
Change a Standard Form to a Custom Self-Hosted Form
To edit the CSS, HTML, or JavaScript, you can change this standard form to a custom self-hosted form. To embed a custom self-hosted form on your website, you must copy the full form code from Bloomerang CRM, edit it outside of Bloomerang CRM, and copy it to your website.
